Participating Sport(s)___________________________________________________________

I hereby give my permission for the registrant to participate in the above-named Sport's practice.

The registrant is not currently under medical care for any condition that would limit his/her ability to safely and fully participate in the above-named Sport(s).

I recognize the possibility of physical injury associated with the above name Sport(s) and in consideration for Providence Athletic Club ("PAC") accepting the registrant for its programs and activities (the "Programs"), I hereby release, discharge and/or otherwise indemnify PAC, its affiliated organizations and sponsors, their employees and associated personnel, including the owners of fields and facilities utilized for the Programs, against any claim by or on behalf of the registrant’s participation in the Programs.

I acknowledge that while PAC does background checks on all coaches and all coaches must sign a statement of faith, PAC is not responsible for the conduct of each coach.

PAC’s policies are designed so that all students are able to compete on a level playing field in a safe, competitive, and friendly environment, free of discrimination. PAC’s rules and regulations are intended to provide every student-athlete with equal opportunities to participate in athletics. A student shall participate based on the gender noted on the student’s certificate of birth. The Certificate of Birth is defined as: 1. It must be an original certified document. 2. It must be obtained from the state, county, or city government in which the student was born. 3. It must include the given and surnames. 4. It must be legible and unaltered. PAC leadership (directors, coaches, managers) will only use pronouns for a player or parent in/ accordance with their biological sex/gender at birth.  All PAC participants will address all other PAC participants according to their biological sex/gender and will not be forced to use "preferred gender" pronouns.

***All those who choose to participate in PAC recognize that PAC Leadership adheres to the PAC Statement of Faith (below) and that these beliefs will be emphasized during practice and games.  PAC understands that participants may hold other beliefs but PAC leadership will not conform to alternative lifestyle beliefs or adjust the way they conduct their training in order to appease someone's alternative lifestyle and beliefs that may not align with the Statement of Faith below.  PAC will allow all who are eligible to participate if they agree to adhere to the PAC standard and character expectations while at practice and at games.  There will be no solicitation or promotion by displaying identifying symbols on personal bags/equipment, at team events, or in team chats of one's alternative lifestyle or gender dysphoria to other PAC participants.

I do grant permission to PAC to authorize publication of my child’s name and/or photograph on the PAC website, in advertisements, and in other social media/publications.

Leadership Statement of Faith

We believe the Bible is the inspired Word of God, without error in the original writings, the complete revelation of God’s will for the salvation of men. All scripture, living and powerful, is profitable to teach true doctrine, for reproof and correction, and to instruct us in a life of righteousness. (2 Peter 1:20-21, 2 Timothy 3:16-17, Hebrews 4:12)

We believe in one God, eternally existent in 3 persons:  Father, Son, and Holy Spirit – the same divine nature (Gen. 1:26, Deut. 6:4, Isaiah 45:18, Matt. 28:19, II Corinthians 13:14, 1 Peter 1:1-2, Jude 20-21).

We believe that salvation is by grace (Ephesians 2:8).  The conditions of salvation are confession of sins and belief in Jesus Christ, who knew no sin, but died a substitutionary death on Calvary for the whole world, being made sin for us (Romans 8:32, John 3:16).  He arose bodily from the grave, ascended into heaven, and is now our Advocate and High Priest at the right hand of the Father (John 11:25-26, 1 Cor. 15:1-4).

We believe that men are justified by faith and are accounted righteous before God only through the merit of our Lord and Savior, Jesus Christ (Romans 10:17, Romans 5:1-2, Romans 3:21-26, Acts 16:3, Acts 20:21).

We believe that our lives are to be an expression of our faith through holy living and righteous works which God planned from the beginning for those who accept His gift of Salvation. (Ephesians 2:1-10)

We believe that God wonderfully and immutably creates each person as male or female. These two distinct, complementary genders together reflect the image and nature of God. (Gen. 1:26-27) Rejection of one’s biological sex is a rejection of the image of God within that person. (Matt 19:4)

We believe that the term “marriage” has only one meaning: the uniting of one man (genetic male) and one woman (genetic female) in a single, exclusive union, as delineated in Scripture. (Gen. 2:18-25.) We believe that God intends sexual intimacy to occur only between a man and a woman who are married to each other. (1 Cor. 6:18; 7:2-5; Heb. 13:4) We believe that God has commanded that no intimate sexual activity be engaged in outside of a marriage between a man and a woman.
